Who we are
The Referral and Assessment Service is a central
resource for referrals into Cranstoun's residential rehabilitation
centres - Cranstoun Oak Lodge and Cranstoun Trelawn
House.
We offer an experienced first contact point for service users and
professionals and aim to provide a fast response for people
requiring residential treatment services.
What we do
The Referral and Assessment Service offers the
following support:
- We will take initial details and complete a referral form with
clients and professionals.
- We will give information about treatment options available to
clients and referrers
- We will arrange and carry out needs and risk assessments at the
service of choice or at other venues if this is not possible
- We will provide pre-admission support to clients including
information on access to funding, contacts for health, housing and
childcare issues
- We will liaise with other agencies involved in the client care
plan to support the care pathway.
We will visit other agencies to promote and describe our
services.

The Referral and Assessment Service is available to
anyone requiring information about, or referral to, Cranstoun's
residential rehabilitation centres - Cranstoun Oak Lodge and
Cranstoun Trelawn House.

Confidentiality
All aspects of confidentiality will be discussed with you during
your contact with us. Ordinarily what you tell us remains
confidential to the team. However, there may be times when we have
to talk to other agencies, which we will discuss with you
first.
